Adding Notes

The Add Note dialog provides a full rich-text editor for creating clinical notes in a patient's chart.

Opening the Add Note Dialog

From the EMR Viewer, click Add Note to open the note editor.

Note Metadata

Before or after composing your note, set the document metadata at the top of the dialog:

Service Dates

  • Service Start — The date the service began (date picker, mm/dd/yyyy format)
  • Service End — The date the service ended (optional, for multi-day services)

Classification

  • Section — Select the chart section (e.g., Progress Notes, Consultation). Use the Show Inactive Sections checkbox to reveal deactivated sections.
  • Plan — Link to a specific treatment plan
  • Event — Link to a specific appointment
  • Provider — The source provider for this note

Document Title

An optional title for the note. This appears in the chart tree and helps identify the document.

Rich Text Editor

The note editor is a full-featured word processor with a ribbon interface organized into tabs:

Home Tab

  • Clipboard — Cut, Copy, Paste
  • Font — Font family, size, increase/decrease size, text case
  • Font Styles — Bold, Italic, Underline, Double Underline, Strikethrough, Double Strikethrough, Subscript, Superscript
  • Colors — Text highlight color, font color
  • Paragraph — Bullets, numbering, multi-level lists, increase/decrease indent, alignment (left, center, right, justify), line spacing (single, 1.5, double)
  • Styles — Quick styles gallery
  • Editing — Find, Replace, Undo, Redo

Insert Tab

  • Pages — Page breaks
  • Tables — Insert and format tables
  • Illustrations — Insert pictures and floating objects
  • Links — Bookmarks and hyperlinks
  • Header and Footer — Add headers and footers
  • Text — Text boxes
  • Symbols — Special characters

Page Layout Tab

  • Margins, Size, Columns — Page setup
  • Line Numbering — Toggle line numbers
  • Page Color — Background color

References Tab

  • Table of Contents — Auto-generated TOC
  • Captions — Figure and table captions

Mail Merge Tab

  • Merge fields — Insert merge fields for mail merge operations

Review Tab

  • Spelling — Spell check
  • Document Protection — Lock document for editing
  • Range Permissions — Set editing permissions for sections

View Tab

  • Layout — Simple, Draft, or Print Layout view
  • Show Options — Toggle formatting marks
  • Zoom — Adjust zoom level

Inserting Snippets

Press F4 or click the Snippet button to open the snippet selector. Snippets are pre-defined text blocks that can be quickly inserted into your note. See Snippets for details on creating and managing snippets.

Inserting Signatures

Press F8 or click the Signature button to insert a provider signature into the note.

Saving

Press F3 or click Ok to save the note to the patient's chart. Press Esc or click Cancel to discard.
